
Consultations rooms are available for use by ophthalmologists on a regular or per session basis. The rooms are equipped with a slit-lamp, tonometer and Snellen chart.

Terms and Conditions for use of consulting rooms
All
practitioners wishing to rent rooms at LEDC whether on an annual or per
session basis will be required to provide photocopies of:
Contemporaneous Curriculum Vitae
GMC or other Professional Body registration
Professional Indemnity Certificate
Criminal Records Bureau (CRB) Disclosure Certificate (where appropriate)
Two (2) professional references
Practitioners are also required provide a written declaration that they are competent to use relevant equipment and are licensed/certified where appropriate.

Enhanced Service - £8,000

Reporting on diagnostic procedures

Free access to educational and social events at LEDC

PER-SESSION BASIS
A commitment to a minimum of 2 sessions per month is required

Payment
All fees are to be paid quarterly, in advance.
Notice Period(s)
Annual Rental Agreements
A minimum period of two (2) months is required by both parties unless a breach of these Terms and Conditions has occurred.
Per Session Rental Agreements
A minimum period of four (4) weeks is required by both parties unless a breach of these Terms and Conditions has occurred.
Review
Both parties will be afforded the opportunity to review the rental and working agreements on a quarterly basis. Options for change can of course be considered here.
LEDC Policies and Procedures
All practitioners will comply with LEDC policies and procedures at all times. A copy of these is available from the Practice Manager or the Directors.
LEDC is registered with the Healthcare Commission, and as such, users will be required to comply with their existing and any future registration standards and/or other requirements.
